29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    The employer failed to protect employees from potential amputation hazards associated with operating unguarded equipment.  This was most recently evident during the inspection of the facility located at 146 n. Maple Ave, Hastings, NE 68901, where the employer has employees operating various pieces of equipment throughout the facility that are not guarded against potential amputation hazards.  Instances include but are not limited to:      i.  Missing point of operation guarding on:    a. In the maintenance shop a RF Model D-91002K drill press, serial number 638080 used by the maintenance     crew when fabricating parts.    b. In the maintenance shop a ACRA Model AM-3A milling machine, serial number 990240 used by the maintenance crew when fabricating parts.    c. In the fabrication shop a Drake Johnson model JH10 horizontal band saw, serial number 185761 used to cut metal parts for fabrication throughout the facility.    d. In the fabrication shop a RF Model 1100000 drill press, serial number 634265 used for drilling metal parts for fabrication throughout the facility.

29 CFR 1910.303(b)(1)(i):     Suitability for installation and use in conformity with the provisions of this subpart;  Note to paragraph (b)(1)(i) of this section: Suitability of equipment for an identified purpose may be evidenced by listing or labeling for that identified purpose.    The employer failed to protect employees from potential electrical hazards associated with using improperly internally fabricated portable lighting in the work place.  This was most recently evident during the inspection of the maintenance shop of the facility located at 146 n. Maple Ave, Hastings, NE 68901, where the employer has employees using a internally fabricated portable hand lamp with multiple safety hazards to provide additional illumination near the lathe in the maintenance shop.  Hazards include but are not limited to:    i.  Light bulb is not protected from accidental contact or breakage by a suitable fixture of lamp holder with a guard as required referenced in 1910.305(a)(2)(ix).     ii.  Portable lamp was internally fabricated with materials and parts that have not been tested and approved in accordance with the National Electric Code.

29 CFR 1910.1200(f)(5):     Except as provided in paragraphs (f)(6) and (f)(7) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the following information:    The employer failed to protect employees from potential chemical hazards associated with using unlabeled secondary containers for chemicals used in production.  This was most recently evident during the inspection of the production floor of the facility located at 146 n. Maple Ave, Hastings, NE 68901, where the employer has employees using unlabeled metal 2 quart containers to store and apply chemicals used in the manufacturing process for PVC pipes.  Chemical exposure includes but is not limited to:      i.  WELD-ON P-68 Low VOC Primer for PVC and CPVC Plastic Pipe.    ii.  WELD-ON 700 Low VOC Cement for PVC Plastic Pipe.
