29 CFR 1910.178(p)(1): Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition 1) The a Crown order picker indutrial truck used on daily basis had a malfanctioning horn. The condition exposed the employees to struck-by hazard. 2) The Toyota industrial truck used by the employees in the warehouse had a malfanctioning horn. The condtion exposed the employees to struck-by hazard.

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: 1) A grey fan located on the first shelf of the rack located on the south wall did not have a guard. The blades exposed the employees to hand and or finger injuries. 2) A white fan located on the floor of the middle isile of the warehouse did not have a guard. The blades exposed the employees to hand and or finger injuries. 3) A blue fan located on the floor of the middle isle did npot have a guard. The blades exposed the employees to hand and fingure injuries.
