29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed: Employees were exposed to entrapment hazards while working in the warehouse as the employer failed to ensure the exit door remained unobstructed:  JOB SITE: 4665 World Parkway Circle, Berkeley, MO 63134, on or about 3/23/2017, on the south wall of the warehouse, an exit door was obstructed with a large industrial fan and an electrical cord.  The workers were exposed to delayed egress hazards.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  JOB SITE: 4665 World Parkway Circle, Saint Louis, MO 63134, on or about 3/23/2017, a forklift operator was observed operating a Crown Forklift, MODEL: 30 SCTT, S/N: 1A125546, with defective and damaged battery power cord.  The power cord was wrapped in green and blue tape which was worn through with exposed wiring.  The workers were exposed to an electrical hazard.  The Stonewheel, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(p)(1), which was contained in OSHA inspection number 534120, citation number 1, item number 1, issued on 9/6/2012 and was affirmed as a final order on 10/1/2012, with respect to a workplace located at 4665 World Parkway Circle, Berkley, MO 63134.

The warehouse receives and stores numerous types of chemicals for the auto industry to include but not limited to battery acid, antifreeze, brake fluid, and other types of motor fluids.
