29 CFR 1910.22(d)(1):     On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.      a)    National Park Service Lassen Volcanic National Park Loomis Entrance Maintenance Bldg. 133 - On or about June 20, 2012 and at times prior thereto:  -  The north and south mezzanines in the maintenance shop have not been evaluated for their load ratings.   -  The middle bay mezzanines on the north, south and east side have not been evaluated for their load ratings.

29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides, or were not guarded with the standard railings (or equivalent) and toeboards; or were not guarded with standard railings (or equivalent) and toeboards.  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a)  National Park Service Lassen Volcanic National Park - Bldg. 133 - On or about June 20, 2012 and at times prior thereto, a section of the Shop's south mezzanine was not protected by a railing on the open side.    b) National Park Service Lassen Volcanic National Park - Bldg. 133 - On or about June 20, 2012 and at times prior thereto, the Middle Bay 's south and north mezzanines did not have a toe board where employees could pass underneath.

29 CFR 1910.25(d)(2)(xv):     No ladder should be used to gain access to a roof unless the top of the ladder shall extend at least 3 feet above the point of support, at eave, gutter, or roofline;  a)  National Park Service Lassen Volcanic National Park - Bldg. 133 - On or about June 20, 2012 and at times prior thereto, a ladder used to access the shop areas' south mezzanine did not extent at least three feet above the point of support to prevent it slipping out from under the employee.

29 CFR 1910.37(b)(5):     Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or identified by a sign indicating its actual use (e.g. closet):  a)  National Park Service Lassen Volcanic National Park - Bldg. 133 - On or about June 20, 2012 and at times prior thereto, the side of the door leading from the shop to the middle bay, the side of the door leading from the middle bay to the shop, and a door on the middle bay's south side that could be mistaken for exit access were not marked with "Not an Exit" or similar designation or identified by a sign indicating their actual use.
