Seven employees based at Building 200 work in both the main building and the detached garage.

One of the conductors was spliced at two locations; at the junction of the splice splitting of one conductor into two and at a location closer to the electrical panel.

The disconnecting switch was located in wood enclosure under a lift up metal lid delaying access to the switch in an emergency.

Below breaker 7, an opening contained a piece of wood, and the area for breakers 19, 20 and 21 was left completely open.

29 CFR 1910.305(f)(1): All conductors used for general wiring were not insulated unless otherwise permitted in this subpart a) National Park Service Whiskeytown National Recreation Area - Bldg. 200 - On or about June 25, 2012,and at times prior thereto, the covering on electrical wiring emerging from the bottom of the main electrical panel for Building 200 was damaged, exposing the wires below. The electrical panel contains the circuit breakers and main disconnect for the Main building and the detached garage. Dust, spider webs and vegetative debris were also located on and adjacent to the deteriorated sheath on the inside of the wooden cabinet where the electrical wires were located.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: a) National Park Service Whiskeytown National Recreation Area - Bldg. 200 - On or about June 25, 2012,and at times prior thereto, two, red, five gallon Jerry Cans of two stroke fuel for portable pumps were not labeled, tagged or marked with the identity and appropriate hazards warning for the fuel. The cans could have been mistaken for regular gasoline or diesel and used in vehicles or other motors not designed to use two stroke fuel causing seizing or explosion hazards. Cans could also have been accidentally filled with gasoline or diesel by one employee and placed in portable pumps believing it was two stroke fuel creating a fire/explosion hazard.

29 CFR 1910.303(e): The manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product may be identified was not placed on the electrical equipment that was used: a) National Park Service Whiskeytown National Recreation Area - Bldg. 200 - On or about June 25, 2012,and at times prior thereto, the main disconnect switch did not have the name of the manufacturer, trademark, or other descriptive markings by which the organization responsible for the product could be identified. It also did not have the voltage, current, wattage or other rating as necessary placed on the equipment. A switch with an on/off and 200 was part of the panel. The staff electrician has no information to determine information about the installation and actual voltage, current, etc. without the markings and could be exposed to electrical hazards during evaluation, correcting electrical issues identified and other wiring activities. Access to area behind the metal covering is not restricted and employees could easily access the wiring behind the panel thinking it lead to a main breaker switch and expose them to the main voltage/current coming into the main disconnect.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: a) National Park Service Whiskeytown National Recreation Area - Bldg. 200 - On or about June 25, 2012,and at times prior thereto, Circuits 6 and 7 were not marked to indicate their purpose at the breaker box. Employees would not know which circuit to disconnect when necessary in an emergency or in case of electrical fire in the structure or detached garage. The staff electrician would not know which breaker(s) to disconnect when working on the electrical system associated with Bldg. 200. Without the labeling of the circuits the potential for either electrical related injury or fire present. Other breakers in the panel were labeled.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installation(s) 600 volts, nominal, or less were used in prohibited manners: Note: Temporary electrical power and lighting installation(s) may only be used as specified by 1910.305(a)(2)(i)(A) through 1910.305(a)(2)(i)(C). a) National Park Service Whiskeytown National Recreation Area - Bldg. 200 - On or about June 25, 2012,and at times prior thereto, an electrical outlet had been spliced into the electrical panel box and never removed. The box hung from wires exiting the main overcurrent panel and had been in place for over 90 days and no remodeling, maintenance or repair of the building or equipment were taking place and there was no emergency taking place as allowed under 1910.305(a)(2)(i)(A) through (C). Employees working in the area located between the structure and garage that is adjacent panel box with temporary outlet could use the outlet while working on projects at the picnic table approximately 4 feet away exposing them to possible electrical burns or electrocution as the outlet wiring had no strain relieve or protection from the wires being damaged, cut or coming into contact with water during rain or snow periods.
