29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked in a conspicuous place in each space to which they related:      a)  U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Fire Warehouse - On or about June 27, 2012 and at times prior thereto, the mezzanine used for storage and an office located above the warehouse office, restroom and lockup room used by the warehouse Manager and other employees was not marked with the approved load rating nor was the load rating for the mezzanine known to ensure it was not a hazard for employees located beneath the mezzanine.    b)  U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 12-2102 - On or about June 27, 2012 and at times prior thereto, the mezzanine used for storage above a storage cage and a designated emergency exit route from the buidling was not marked with the approved load rating nor was the load rating for the mezzanine known to ensure it did not present a hazard for employees using the exit door or obtaining stored materials located underneath the mezzanine.    c)   U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 12-2102 - On or about June 27, 2012 and at times prior thereto, the mezzanine used for storage of fleet maintenance parts above the workshop area and welding station was not marked with the approved load rating nor was the load rating for the mezzanine known to ensure it did not present a hazard for employees working underneath the mezzanine.    d)   U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 13-2206 - On or about June 27, 2012 and at times prior thereto, the mezzanines used for storage of wood above rooms and areas used by employees in the sign shop were not marked with the approved load rating nor were the load ratings for the mezzanines known to ensure they did not present a hazard for employees in the rooms below.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a)   U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 13-2206 - On or about June 27, 2012 and at times prior thereto, the mezzanines used to store wood and accessed using ladders were not guarded with standard railings (or equvalent) and standard toeboards.  The mezzanines were over four feet above the lower level.      b)   U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 13-2207 - On or about June 27, 2012 and at times prior thereto, the landing of the entry door into the Project storage Bldg. did not contain a standard railing on one side to prevent employees from falling to the level below.

The stairs lead to the door of the Project Storage Buidling.

An additional stairway, on the outside of the building, was present but was not accessible or designated as an exit route as the room leading to the stairway was used as a managers office.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:      a)  U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Fire Warehouse - On or about June 27, 2012 and at times prior thereto, an exit sign was not posted on/at the door leading from the Fire Warehouse into the fire managers office to indicate that the only exit door and path to the exit door in the Fire Warehouse was inside the managers office.      b)  U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 12-2102 2nd floor Fleet Maintenance kitchen/locker room/officex - On or about June 27, 2012 and at times prior thereto, signs indicating the path to the nearest designated exit route were not posted at the stairs or after descending the stairs into the central hallway and around to the designated exit doors when the path to the nearest exit door(s) were not immediately apparent or visible and more than one non designated exit door was present on the first floor from the hallway.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:       a)  U.S. Forest Service Lassen National Forest Susanville 5th Street Warehouse and Shops (Yard) - Bldg. 12-2102 - On or about June 27, 2012 and at times prior thereto the Jet Grinder Model JBG-6A with Serial Number 1120405 left and right work rests were not adjusted to within one-eigth inch or less of the grinding wheel.  The left side of the left work rest was mesured at at least 7/16th inch from the wheel and the right side of the left work rest was measured at three eights inch from the wheel.  The left side of the right work rest was measured at three eights inch from the wheel and the right side of the right grinding wheel was measured at five sixteenth inch from the wheel.

The left tongue guard was mesured at at least 1 5/8 inchs from the wheel and the right tongue guard was measured at 7/8 inch from the wheel.
