29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which were supplied and securely affixed by the owner of the building, or the duly authorized agent, in a conspicuous place in each space to which they related:      a)  U.S. Forest Service Lassen National Forest Eagle Lake Ranger District - Warehouse - On or about June 28, 2012 and at times prior thereto, the Timber mezzanine used for storage including, but not limited to, lockers, work surfaces with storage underneath and a wood enclosure containing additional lockers and supplies for Timber staff activities was not marked with the approved load rating nor was the load rating for the mezzanine known to ensure it was not a hazard for employees while on the mezzanine and accessing the area underneath the mezzanine.     b)  U.S. Forest Service Lassen National Forest Eagle Lake Ranger District - Warehouse - On or about June 28, 2012 and at times prior thereto, the Fire mezzanine used by Fire, Range, Archeology and Admin for storage including, but not limited to, lockers, wood storage cages with stored supplies and a file cabinet was not marked with the approved load rating nor was the load rating for the mezzanine known to ensure it was not a hazard for employees accessign the mezzanine or in the storage spaces underneath the mezzanine.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":      a)  U.S. Forest Service Lassen National Forest Eagle Lake Ranger District - Warehouse - On or about June 27, 2012 and at times prior thereto, an exit sign was not posted on/at the designated exit door for the Eagle Lake Ranger District Warehouse to ensure employees were able to locate the exit in an emergency.  The warehouse has no windows and other openings, doors and areas could be mistaken for the exit if it does not have an exit sign.  An exit sign should be clearly visible from all areas of the warehouse.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:      a)  b)  U.S. Forest Service Lassen National Forest Eagle Lake Ranger District - District Office - On or about June 28, 2012 and at times prior thereto, sign(s) indicating the path to the nearest designated exit route were not posted at the "T" of the hallway containing offices and the entrance/reception are of the District Office indicating the direction to the nearest exit at either the entrance or the far end of the hallway.  An exit sign at the end of the hallway was not readily visible from any area of the hallway, until at the exit door, as it was attached at ceiling height.  Hallway doors are glass panelled and light can be seen through them during the day.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment:      a)  a)  U.S. Forest Service Lassen National Forest Eagle Lake Ranger District - Warehouse - On or about June 28, 2012 and at times prior thereto, the warehouse did not have the required 3.0 foot depth of clearance in front of the electrical disconnect.  A frezzer was located in front of the disconnect resulting in difficult access and an inability to maintain the equipment.
