29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards:      a)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Timber Building - On or about June 28, 2012 and at times prior thereto, the landing of the Timber Building was uneven with holes and dips that does not facilitate cleaning of the floor and steps.  The surface also creates a tripping hazard.

29 CFR 1910.23(a)(1): Every stairway floor opening shall be guarded by a standard railing constructed in accordance with paragraph (e) of this section. The railing shall be provided on all exposed sides (except at entrance to stairway). For infrequently used stairways where traffic across the opening prevents the use of fixed standard railing (as when located in aisle spaces, etc.), the guard shall consist of a hinged floor opening cover of standard strength and construction and removable standard railings on all exposed sides (except at entrance to stairway):      a)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Wellness/A House - On or about June 28, 2012 and at times prior thereto, the stairway landing at the front door did not have the required railing on all exposed sides.  The left side railing (facing the doorway) stopped 13 inches from the doorway wall.    b)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Wellness/A House - On or about June 28, 2012 and at times prior thereto, the right side (facing the doorway) railing stopped 17 inches from the doorway wall.    c)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Wellness/A House - On or about June 28, 2012 and at times prior thereto, the stairway landing at the back door did not have the required railing on all exposed sides.  The left side (facing the doorway) stopped 9 inches from the doorway wall.    d)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Fire Office - On or about June 28, 2012 and at times prior thereto, the stairway railing was not guarded by a standard railings containing a mid-rail.    e)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Tree Refrigerator - On or about June 28, 2012 and at times prior thereto, the stairway railings were not provided on all exposed sides of the steps.  The railing did not extend to the bottom of the stairs leaving exposed sides on the first two steps.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:      a)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Timber Bldg.- On or about June 28, 2012 and at times prior thereto, the rise on the steps to the front door of the Timber Bldg. were uneven creating potential for falls.  The measured rise(s) were: bottom step 3 7/16th inches; middle step 3 10/16th inches and the top step/landing 6 inches tall.  The concrete on the steps was also eroded and uneven making the actual height of each steps vary across its length.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:      a)  U.S. Forest Service Lassen National Forest Lake Almanor Ranger District - Storage/Warehouse - On or about June 28, 2012 and at times prior thereto, the designated exit route along the hallway from the storage rooms to the designated exit door were not kept free and unobstructed.  The path was partially blocked in some areas with a lawnmower, cart and filled orange storage/water bags and completely blocked in most areas with snow blower, wood and paper debris, ATV, garbage cans, stored wood timbers, ice cooler, desks, and boxes of various supplies preventing ready emergency egress to the emergency exit door.
