29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: (a) The North West crane had deficiencies from the keyway being worn, the pinion coupling was loose or damaged and was not corrected before the operation of the crane was resumed. (b) The North East crane had deficiencies from the directional contactors being damaged and was not corrected before the operation of the crane was resumed. The load block sheave spacers are missing, and the second speed contactors are damaged. (c) The South East crane had deficiencies from the equalizer sheave spacers are missing, and there are pitted and burnt contacts. The load block sheave spacers are missing, and the second speed contactors are damaged. (d) The South West crane has an oil leak and was not corrected and the crane has not been taken out of service. The sheave is damaged, and sheaves do not meet manufacturer's specifications. The conductor bar insulation is missing or damaged. The resistor guard is damaged. The bridge machinery guard is missing or damaged.

(b) Two 480 volt breaker boxes were in the on position on the south wall labelled "charger". It was not evident what the breaker boxes are actually for as there are no forklift chargers in the area.

The hazard communication program needs to include, labeling and other forms of warning, how material safety data sheets are to be obtained and maintained, and employee information and training.
