29 CFR 1910.141(d)(2)(i): Lavatories shall be made available in all places of employment. The requirements of this subdivision do not apply to mobile crews or to normally unattended work locations if employees working at these locations have transportation readily available to nearby washing facilities which meet the other requirements of this paragraph. a) At the facility, lavatory facilities were not available to employees working in the area of the rail office who work with containers of solid waste, garbage, and human sewage sludge.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a) At the establishment, industrial truck operators were operating the Taylor TEC-950 industrial truck in a mode that bypassed the truck's indicator system that notified the operator of the status of the twistlock lifting pins exposing employees to struck-by hazards. Daily inspection forms indicated that the indicator system was in bypass mode and the industrial truck had not been taken out of service.
