29 CFR 1910.132(d): Hazard assessment and equipment selection.  29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:  29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  On or about April 9, 2018, the employer failed to select and require the use of personal protective equipment, including but not limited to leg protection for employees performing required duties that involved welding. Welders and welder's helpers job positions performed welding on a daily basis. Due to this condition, employees were exposed to thermal contact and increased risk for second and third degree burns.

29 CFR 1910.132(d): Hazard assessment and equipment selection.  29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about April 9, 2018, the employer failed to perform and certify the performance of the personal protective equipment hazard assessment required by this part. Welders and welder's helpers job positions performed welding on a daily basis. Due to this condition, employees were exposed to thermal contact and increased risk for second and third degree burns.

29 CFR 1910.303(g): 600 Volts, nominal, or less. This paragraph applies to electric equipment operating at 600 volts, nominal, or less to ground.  29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about April 9, 2018, the working space about two electrical subpanel boxes and a "trough" style, horizontal feeder box located near the maintenance shop entrance door was not maintained free of stored items. A wheeled tool cabinet and vehicle light assembly were stored within the three feet working space required by Table S-1 from this part.
