29 CFR 1910.252(a)(1)(i): When the object to be welded or cut could not be readily moved, all movable fire hazards in the vicinity were not taken to a safe place: a) Welding Area: Employees, Welders, were exposed to fire hazards when welding galvanized steel, stainless steel, and aluminum in welding booths with cardboard sheets hanging on the booth curtain to block the welding flash from the other booths, on or about 08/08/2012. b) Welding Area: Employees, Welders, were exposed to fire hazards when welding galvanized steel in a welding booth that had a stack of empty cardboard boxes in the booth, on or about 08/08/2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1000(b)(1): Employee(s) were exposed to an airborne concentration of zinc oxide listed in Table Z-2 in excess of the 8 hour Time Weighted Average concentration of 5 milligram per cubic meter of air: a) Welding Area: An employee, Welder, while welding galvanized steel cart corral bases was exposed to zinc oxide at an 8 hour time-weighted average (TWA) of 6.47 milligrams per cubic meter of air. This level is 1.29 times the permissible exposure limit of 5 milligrams per cubic meter of air. This exposure occurred over a 437 minute sampling period on 08/08/2012. A zero exposure is assumed for the 43 minutes not sampled. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) Welding Area: An employee, Welder, while welding galvanized steel cart corral bases was exposed to zinc oxide at an 8 hour time-weighted average (TWA) of 6.47 milligrams per cubic meter of air. This level is 1.29 times the permissible exposure limit of 5 milligrams per cubic meter of air. This exposure occurred over a 437 minute sampling period on 08/08/2012. A zero exposure is assumed for the 43 minutes not sampled. Effective engineering or administrative controls were not utilized to reduce the welders zinc oxide exposures below the permissible exposure limit, on or about 08/08/2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records. Feasible abatement methods include but are not limited to: 1) Provide localized exhaust ventilation to capture metal fumes at the point of generation and exhaust them outside the work environment. 2) Use administrative controls such as limiting employee exposure to welding fume by limiting the amount of time employees would weld in a day. This cound be accomplished by rotating employee to a job where there would not be exposure to welding fumes. ABATEMENT STEPS ARE AS FOLLOW: STEP 1 As an interim measure, effective respiratory protection shall be provided to and used by exposed employees until feasible and/or administrative controls can be implemented or whenever such controls fail to reduce employee exposure to within permissible exposure limits. STEP 2 - A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposures to hazardous substances as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the dates required by this citation: 1. Evaluation of engineering control options; 2. Selection of optimum control method and completion of design; 3. Procurement, installation and operation of selected control measures; 4. Testing and acceptance or modification/redesign of controls. All proposed control measures shall be evaluated for each particular use by a competent industrial hygienist or other technically qualified person. STEP 3 - Abatement shall have been completed by the implementation of feasible engineering and/or administrative controls upon verification of their effectiveness in achieving compliance.

As required by 1910.252(c)(3) local exhaust hoods or booths were not utilized when employees were welding on metals containing a zinc coating, on or about 08/08/2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
