29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a) Assembly  Area: Two flexible cords were run from the ceiling outlet to both sides of the Assembly Area to provide power for multiple grinders and polishers in lieu of fixed wiring, on or about June 11, 2014.    b) Wysong Press Brake Area: A flexible cord was run from the ceiling outlet to the Wysong Press Brake in lieu of fixed wiring, on or about June 12, 2014.    No abatement certification or documentation required.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Assembly and Welding Areas: Employees were provided Defend Form Fit Mask (MK-1206) to wear on a voluntary basis and employees were not provided with the information in Appendix D, on or about June 11, 2014.  No abatement certification or documentation required.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) Welding and Assembly Areas: Employees welding aluminum and using various gases were not provided with information on the new labeling system and safety data sheet structure, on or about June 11, 2014.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
