29 CFR 1910.23(a)(3): The employer did not ensure that every hatchway and chute floor opening was guarded by either a hinged floor opening cover of standard strength and construction equipped with standard railings or permanently attached thereto so as to leave only one exposed side, or by a removable railing with toeboard on not more than two sides of the opening and fixed standard railings with toeboards on all other exposed sides: Split-Leg Area, northwest corner of floor, south area of worksite: On 05/17/2012, employees engaged in grain elevator support activities were exposed to trip hazards from a closed, hinged floor cover that contained a 13 1/2 inch wide by 16 inch long cavity.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide: Northwest interior, top floor area of elevator head house building, south area of the worksite: On 05/17/2012, employees engaged in grain elevator support activities were exposed to trip hazards from uncovered floor holes that measured 5 1/2 inches wide by 20 inches long.

29 CFR 1910.23(b)(3): 29 CFR 1910.23(b)(3): Every window wall opening at a stairway landing, floor, platform, or balcony, from which there was a drop of more than 4 feet, and where the bottom of the opening was less than 3 feet above the platform or landing, was not guarded by standard slats, standard grill work (as specified in paragraph (e)(11) of this section), or standard railing: Northwest interior, top floor area of elevator head house building, south area of the worksite: On 05/17/2012, employees engaged in grain elevator support activities were exposed to fall hazards from the unguarded wall opening. The unguarded opening was 41 inches long, 28 inches wide; with a fall distance of 90 feet to the ground.
