29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points. Northwest area of Elevator Head House, south area of workplace, exit access: On 05/17/2012, employees were exposed to fall hazards when traveling to/from the interior wall mounted roof access ladder using the ladder exit access platform. The ladder exit access platform was 16 inches wide, 62 inches long, with a height of 88 inches above the interior top floor; and used by employees to support maintenance of roof mounted communication equipment.

29 CFR 1910.37(a)(2): The path of travel used to reach the nearest exit was not effectively shielded from the high hazard location by suitable partitions or other physical barriers: Northwest area of Elevator Head House, south area of workplace, exit access: On 05/17/2012, employees were exposed to fall hazards while traveling upon the ladder exit access platform through a high hazard area. The platform was not provided with suitable barriers to prevent a 90 foot fall of an employee from the exit access platform into the high hazard- open elevator shaft area.
