29 CFR 1910.178(p)(1):Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s): 509 Bankhead Highway, Winder, GA (Warehouse) - Employees were exposed to struck-by hazard. Employees were operating the Hyster forklifts to transport heavy loads of metal/steel material equipment; doing so with an inoperable back-up alarm on the forklifts. The forklifts are being operated in a heavy traffic area where employees and management accesses on a daily basis, on or about 08/09/12. a. Hyster #120 sit-down forklift b. Hyster #155 sit-down forklift

29 CFR 1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: 509 Bankhead Highway, Winder, GA (Warehouse) - The employer developed specific written lockout/tagout procedures; however, the employer has not performed a periodic inspection and/or annual inspection of the procedures to verify if any changes had been made to the lockout/tagout procedures, on or about 08/09/12.
