Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure energy control procedures were developed, documented, and utilized to prevent caught by and amputation hazards while changing the tools on the Amada Vipros 255 CNC Punch.    b.) Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure energy control procedures were developed, documented, and utilized to prevent amputation hazards while changing blades on the Lucas Circular Sheer.    c.) Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure energy control procedures were developed, documented, and utilized to prevent amputation hazards and changing out dies on the Niagra full revolution mechanical power press.

Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure employees were adequately trained to understand the safe application, usage, and removal of energy controls while performing maintenance and setup activities on equipment, such as but not limited to, Amada Vipros 255 CNC Punch, Lucas Circular Sheer, and the Niagra full revolution mechanical power press. Employees are exposed to caught by hazards from unexpected startup or energization of the equipment.

Miller and Mapco Hi Tech Fab, LLC located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure die setting procedures were established which would protect the die setting employees from the downward motion of the mechanical power presss ram. Employees were not protected from the downward motion of the ram through the use of adequate machine guarding or the isolation of the energy sources by the implementation of lockout procedures.

Employees were exposed to caught by hazards while using the drill press to drill holes in metal parts for airline food carts.	    b.) Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure the Wysong Hydraulic Press Brake was properly guarded. Employees were exposed to amputation hazards while bending metal parts.    c.) Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure the Adira Hydraulic Press Brake was properly guarded. Employees were exposed to amputation hazards while bending metal parts.    d.) Miller and Mapco Hi Tech Fab, LLC, located at 125 S. Mill St. Lexington, OH; On or about August 14, 2012, the employer did not assure the nut end of the shaft on the Baldor Buffer Wheel was properly guarded. Employees were exposed to caught by hazards while buffing metal parts.
