29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a.) Miller & Mapco HiTech Fab, LLC located at 125 South Mill St. Lexington, OH; On or about November 7, 2013, the employer did not assure employees used electrical safety related work practices when accessing the 110V control panel of the #9 Minster Mechanical Power Press to switch from run mode to inch mode when changing dies, in that proper PPE was not worn. Employees were exposed to electrical shock and arc flash hazards from the 110V equipment.

Miller & Mapco HiTech Fab, LLC located at 125 South Mill St. Lexington, OH 44904; On or about November 18, 2013, the employer did not assure employees wore personal protective equipment, including but not limited to, insulated gloves with leather gloves while switching from run to inch mode while changing dies in a 110V control cabinet for the Minster #9 Mechanical Power Press, exposing employees to electrical shock and arc flash hazards.
