29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees' ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) Miller and Mapco Hi Tech Fab, LLC - Lexington, Ohio: On or about September 5, 2012, the employer  failed to provide medical evaluations to those employees who were required to wear half mask tight-fitting respirators and dust masks while grinding and sanding. Employees were exposed to hazards from aggravation of pulmonary issues, such as, but not limited to, asthma and emphysema.
