29 CFR 1926.451(b)(1):  Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:    a)  At 2575 Westbury Court Auburn, Alabama :  On or about August 15, 2012 and at times prior; the employer exposed his employees to fall hazards in that employees were allowed to work from the first level of a three bucks high scaffold, located at the front of the house which was not fully planked exposing them to falls of 7.8 feet to the ground below.     b)  At 2575 Westbury Court Auburn, Alabama :  On or about August 15, 2012 and at times prior; the employer exposed his employees to fall hazards in that employees were allowed to work from the second level of a three bucks high scaffold, located at the front of the house which was not fully planked exposing them to falls of 14.3 feet to the ground below.    c)  At 2575 Westbury Court Auburn, Alabama :  On or about August 15, 2012 and at times prior; the employer exposed his employees to fall hazards in that employees were allowed to work from the first level of a two bucks high scaffold, located at the rear of the house which was not fully planked exposing them to falls of 7.0 feet to the ground below.    d)  At 2575 Westbury Court Auburn, Alabama :  On or about August 15, 2012 and at times prior; the employer exposed his employees to fall hazards in that employees were allowed to work from the first level of a three bucks high scaffold, located at the side of the house which was not fully planked exposing them to falls of 6.9 feet to the ground below.    e)  At 2575 Westbury Court Auburn, Alabama :  On or about August 15, 2012 and at times prior; the employer exposed his employees to fall hazards in that employees were allowed to work from the second level of a three bucks high scaffold, located at the side of the house which was not fully planked exposing them to falls of 12.9 feet to the ground below.

29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a)  At 2575 Westbury Court Auburn, Alabama:  On or about August 15, 2012 and at times prior; the employer failed to ensure scaffolding used at the rear of the house had base plates installed to prevent damage to the scaffold.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    a) At 2575 Westbury Court Auburn, Alabama:   On or about August 15, 2012 and at times prior; the employer failed to provide a ladder for employees to use to gain access to and from the scaffold located at the front of the house exposing them to falls of 14.3 feet to the ground below.    b) At 2575 Westbury Court Auburn, Alabama:   On or about August 15, 2012 and at times prior; the employer failed to provide a ladder for employees to use to gain access to and from the scaffold located at the side of the house exposing them to falls of 12.9 feet to the ground below.    c) At 2575 Westbury Court Auburn, Alabama:   On or about August 15, 2012 and at times prior; the employer failed to provide a ladder for employees to use to gain access to and from the scaffold located at the rear of the house exposing them to falls of 7.0 feet to the ground below.
