29 CFR 1910.94(c)(3)(ii):  Unobstructed walkways shall not be less than 6 1/2 feet high and were not maintained clear of obstruction from any work location in the booth to a booth exit or open booth front. In booths where the open front is the only exit, such exits shall be not less than 3 feet wide. In booths having multiple exits, such exits shall not be less than 2 feet wide, provided that the maximum distance from the work location to the exit is 25 feet or less. Where booth exits are provided with doors, such doors shall open outward from the booth.        a)  Paint Booth:  On or about 08/02/12 and times prior thereto, employees were not provided accessibility to the exit door due to the bus blocking the door during spray painting operations.    b)  Paint Booth:  On or about 08/02/12 and times prior thereto, employees did not have accessibility to the exit door of the paint booth because an object was blocking the closed door which opens outward.

29 CFR 1910.165(e):  Manually operated actuation devices for use in conjunction with employee alarms were not unobstructed, conspicuous and readily accessible:         a)  Paint Booth:  On or about 08/02/12 and times prior thereto, the employer did not permit employees a safe access to the manual fire alarm which is located outside and next to the exit door.  During spray painting; the 45 feet in length Standard Motor Coach (bus) blocks the exit door which is located on the west side; and employees have approximately 25 to 45 feet to reach the manual fire alarm.

29 CFR 1910.134(h)(2)(i):  Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:        a)  Paint Booth; On or about 08/02/12 and times prior thereto, employee respirators were contaminated with paint mist and were hanging on the wall.     b) Prep Area; On or about 08/02/12 and times prior thereto, employee respirator was contaminated with dust and stored on cart along with chemicals such as primer, thinner, and reducer which was located outside the spray booth.

29 CFR 1910.138(a):  The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes:        Paint Booth; On or about 08/02/12 and times prior thereto, employees hands were exposed to paint during spray painting when wearing latex gloves that tear easily while working.  The recommended protective gloves per the MSDS are the use of solvent resistant gloves such as nitrile rubber.
