29 CFR 1910.151(c):   Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not be provided within the work area for immediate emergency use:  On or about February 17, 2016, and at times prior thereto, employees were exposed to chemical burns, while using corrosive materials such as, but not limited to, Auto Wax Red Hot All Purpose Cleaner, without a functioning eyewash in the shop.

29 CFR 1910.305(b)(1)(ii):   Unused openings in cabinets, boxes, and fittings were not effectively closed:  On or about February 17, 2016, and at times prior thereto, an employee was exposed to the hazards of electricity, while switching on and off the breaker switch to the air compressor, with an unused opening in the panel box not being covered.

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to use and at least annually thereafter:  On or about February 17, 2016, and at times prior thereto, an employee was exposed to health hazards from chemicals used in painting operation such as, but not limited to, Lusid Technologies EP210LG Epoxy Primer, as he was not fit tested annually for respirator use.  Star Shuttle & Charter was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 588738, citation number 1, item number 6, and was affirmed as a final order on February 5, 2013, with respect to a workplace located at 8715 Airport Blvd, San Antonio, TX 78216.

29 CFR 1910.134(c)(1):  In the workplace where respirators were necessary to protect the health of the employee and were required by the employer, the employer did not implement and maintain a written respiratory protection program with worksite-specific procedures.  The program was not updated as necessary to reflect those changes in workplace conditions that affected respirator use:   On or about February 17, 2016, and at times prior thereto, an employee was exposed to health hazards from chemicals used in painting operations, in that the respiratory protection program did not contain procedures specific to the paint booth operation at the worksite.

29 CFR 1910.134(d)(3)(iii)(B)(2):  There was no ESLI (end-of-service-life indicator) on the air-purifying respirator appropriate for conditions in the employer's workplace, and the employer did not implement a change schedule for canisters and cartridges based on objective information or data that ensured canisters and cartridges are changed before the end of their service life. The employer did not describe in the respirator program the information and data relied upon and the basis for the canister and cartridge change schedule and the basis for reliance on the data:  On or about February 17, 2016, an employee was exposed to health hazards from chemicals used in painting operations such as, but not limited to Lusid Technologies EP210LG Epoxy Primer, as there was no respirator cartridge ESLI or logs to track usage and mandatory respirator cleaning procedures (Reference 29 CFR 1910.134 Appendix B-2).
