29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) North east deck: On August 9, 2012 a completed guardrail system was not in place to protect employees performing framing and remote control crane activites. Employees were exposed to a 10 foot fall hazard to the deck below. b) West deck: On August 9, 2012 an employee performed remote control crane activities without a fall protection system in place. The employee was exposed to a 10 foot fall hazard to the deck below. Note) Abatement certification is NOT required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) North east deck: On August 9, 2012 an extension ladder used for acces to/from the deck area, was positioned such that the side rails and top rung of the ladder were at or below the deck level. Note) Abatement certification is NOT required for this item.
