29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) BYU Student Housing, South end level 3, Bldg. A: On or about November 20, 2014, a completed guardrail system was not in place to protect employees performing framing and accessing/egressing the stairs. Employees were exposed to an approximate 10 foot 6 inch fall hazard to the deck below. b) BYU Student Housing, North side 3rd floor of building 3: On or about November 20, 2014, and at times prior thereto, two employees were working near a wall opening that was approximately 18 feet to the ground. Note) Abatement certification IS required for this item. The Star Mountain Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 591358, citation number 1, item number 1 and was affirmed as a final order on November 2, 2012, with respect to a workplace located at 333 West 6th South, Asbury Apartments, Rexburg, Idaho 83440.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) BYU Student Housing, South end of Building A: On or about November 20, 2015, and at times prior thereto, employees utilized an approximate 24 inch step up/down from a unfinished concrete foundation floor as a means of access/egress to the south stairwell. Note) Abatement certification IS required for this item.
