29 CFR 1910.119(f)(1)(i)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting normal operation activities involved in each covered process consistent with the process safety information: a) On 08/17/12, the employer's operating procedures for packaging CXA Wetcake did not include instructions minimizing the potential for igniting CXA Wetcake. CXA Wetcake is a flammable material. Methods to minimize the possibility of igniting CXA Wetcake while discharging CXA Wetcake from the Rosenmund Filter into 55 gallon drums includes, but is not limited to: 1. Grounding the 55 gallon drum. 2. Inerting the 55 gallon drum before filling. 3. Insuring there is not an air gap between the 55 gallon drum and the discharge chute. No Abatement Documentation or Abatement Certification Required.

29 CFR 1910.119(j)(2): The employer did not implement written procedures to maintain the ongoing integrity of process equipment: a) The employer had not replaced or inspected the pressure relief valves on the Methyl Chloride Tank in accordance with their Mechanical Integrity Program. The employer's Mechanical Integrity Program requires pressure relief valves to be inspected or replaced every 10 years. The pressure relief valves on the Methyl Chloride Tank were installed 11/15/2000 or 10/29/2001. As of 08/28/2012, the employer had not replaced or inspected nor scheduled the replacement or inspection of the Methyl Chloride Tank pressure relief valves. Abatement Documentation and Abatement Certification required.

The operator was stationed at the discharge chute of the Rosenmund Filter in Building 52. No Abatement Documentation or Abatement Certification Required.
