Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard from standing on the railing of a scissors lift:    a)  Employees were observed climbing and standing on the toprail and midrail of the aerial platform of a Genie GS-3246 scissors lift.     Among other methods, feasible and acceptable methods of abatement to correct this hazard include, but are not limited to the following:    1) Conform to the Genie Operators Manual safety rules on maintaining a firm footing on the platform floor at all times.    2)  Conform to the American National Standard Institute (ANSI) for Self-Propelled Elevating Work Platforms, ANSI/SIA A92.6-2006 Paragraph 8.10 Section (9).    All Steel Iron Works, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard Section 5(a)(1) of the Occupational Safety and Health Act of 1970, which was contained in OSHA inspection number 107984, citation 1, item number 1, issued on December 8, 2011, and was affirmed as a final order on or about January 3, 2012, with respect to a workplace located at 1430 N. Meacham Road, Schaumburg, Illinois.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.300(b)(1):  Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:    a) An employee was observed using an electric Milwaukee 4.5 heavy-duty hand grinder without a guard.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.303(d): Section 8.5.6 American National Standards Institute B7.1-1970, Safety Code for the Use, care, and protection of Abrasive Wheels as adopted by 29 CFR 1926.303(d): It shall be the users responsibility to follow any special operating instructions furnished by the machine builder:    The employer provided and allowed for employee use at the job site an electric Milwaukee 4.5 heavy-duty hand grinder, equipped with a 6 inch SAIT A60S-BF general purpose cutting wheel rated for a maximum RPM of 10,200.     a) According to Milwaukees operators manual the rated speed of the accessory must be at least equal to the rated speed of the tool. The rated speed of the grinder was 11,000 RPM and the wheel was rated for a maximum RPM of 10,200.     b) According to Milwaukees operators manual the outside diameter and thickness of the accessory must be within the capacity of the power tool. The grinder is designed for 4.5 inch wheels and wheel used was 6 inches in diameter    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.755(a)(1) Columns were not anchored by a minimum of 4 anchor rods (anchor bolts):    a) The employer did not ensure that the two columns in the center of the building under construction were anchored by 4 bolts. The two columns had 3 bolts each.    No abatement certification or documentation required for this item.
