29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On July 19, 2017, employees were exposed to fall hazards while using an A-frame step ladder without its metal spreaders or locking devices in the open position. b) On July 19, 2017, employees were exposed to fall hazards while working from the top step of an A-frame step ladder. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) On July 19, 2017, employees were exposed to fall hazards from a 4th story deck while performing steel erection activities. All Steel Iron Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.761(a)(1) which was contained in OSHA inspection number 1062466, citation 1, item 1, and was affirmed final order on June 1, 2015, with respect to a workplace located at 19 W. Ogden Ave., Westmont, IL 60559. All Steel Iron Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.761(a)(1) which was contained in OSHA inspection number 107984, citation 1, item 2, and was affirmed final order on January 3, 2012, with respect to a workplace located at 1430 N. Meacham Rd., Schaumburg, IL 60173. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1428(a)(3): The employer did not make the documentation for whichever option is used available at the site while the signal person is employed by the employer. The documentation did not specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section: a) On July 19, 2017 the employer did not have training documentation available onsite for the crane signal person. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
