29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a)  Mundo Roofing at 1905 79th Ave. in Greeley, CO:  On August 29, 2012, the employer did not ensure that employees were protected from injury in that seven employees were working on the first story of a 5:12 pitch roof without utilizing a fall protection system. This condition exposed employees to a fall hazard of 10.5 feet.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Mundo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) which was contained in OSHA Inspection Number 107365, Citation 1, Item 2, issued on October 14, 2011 and became Final Order on October 27, 2011 with respect to a workplace located at 7415 Triangle Dr. in Fort Collins, CO.    Mundo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) which was contained in OSHA Inspection Number 313722977, Citation 1, Item 2, issued on October 21, 2010 and became Final Order on November 12, 2010 with respect to a workplace located at 23986 Currant Dr. in Golden, CO.    Mundo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) which was contained in OSHA Inspection Number 313420200, Citation 2, Item 2, issued on August 6, 2010 and became Final Order on August 31, 2010 with respect to a workplace located at 15847 E. 99th Pl. in Commerce City, CO.    Mundo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) which was contained in OSHA Inspection Number 313717365, Citation 2, Item 1, issued on February 19, 2010 and became Final Order on March 11, 2010 with respect to a workplace located at 1515 Shortgrass Ct. in Castle Rock, CO.    Mundo Roofing was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926. 501(b)(13) which was contained in OSHA Inspection Number 313715294, Citation 1, Item 3, issued on November 27, 2009 and became Final Order on December 7, 2009 with respect to a workplace located at 7335 W. Jewell Ave. in Lakewood, CO.
