29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:         a)  On or about November 14, 2013, employees were exposed to head injuries as they worked without head protection in areas where shingles and other roofing waste were being thrown from upper levels.      Mundo Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.100(a) or its equivalent which was contained in OSHA inspection 313715294, Citation 1, Item 1, issued November 27, 2009, with respect to a workplace located at 7335 W. Jewell Avenue, Lakewood, CO 80232.  This citation became final on December 7, 2009.

29 CFR 1926.102(a)(1):  Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:      a)  On or about November 14, 2013, employees were exposed to eye injuries while they used pneumatic nail guns without eye protection.     Mundo Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1) or its equivalent which was contained in OSHA inspection 313717365, Citation 2, Item 2, issued February 19, 2010, with respect to a workplace located at 14976 E. 12th Avenue, Aurora, CO 80011.  This citation became final on March 11, 2010  Mundo Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.102(a)(1) or its equivalent which was contained in OSHA inspection 313715294, Citation 1, Item 2, issued November 27, 2009, with respect to a workplace located at 7335 Jewell Ave, Lakewood, CO 80232.
