The system must be attached to a structural member within the personnel platform. When working over or near water, the requirements of  1926.106 apply.    (a) The employer did not ensure that employees were using a personal fall arrest system that was anchored to a strutural member of a personnel platform. The employees were working from the platform in order to drill holes at different elevations above a cofferdam. The employees were exposed to a fall hazard.

The retractable lanyards were connected to the cables used to support the personnel platform that was suspended by a crane. The employees were exposed to a fall hazard.
