29 CFR 1926.1431(k)(10)(i): Except over water, employees occupying the personnel platform must be provided and use a personal fall arrest system. The system must be attached to a structural member within the personnel platform. When working over or near water, the requirements of 1926.106 apply. (a) The employer did not ensure that employees were using a personal fall arrest system that was anchored to a strutural member of a personnel platform. The employees were working from the platform in order to drill holes at different elevations above a cofferdam. The employees were exposed to a fall hazard.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: (a) The employer did not ensure that employees were using an anchor point that was independent of the cables being used to support a personnel platform. The retractable lanyards were connected to the cables used to support the personnel platform that was suspended by a crane. The employees were exposed to a fall hazard.
