29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about August 29, 2012, at 4200 North Mesa St, El Paso, Texas, two employees were observed working on a three tier scaffold without a fully planked or decked platform on all working levels.  Employees were exposed to a fall of approximately 12 to 18 feet to the ground below.
