29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm in that employees were exposed to the hazard of being ejected or falling from the operators seat:    Warehouse receiving, Bld 148: On or about 04SEP12, an employee was observed to operate a Yale propane forklift without the seat belt fastened.    Abatement Note:    Among other methods, one acceptable means of abatement would be to use the seat belt in accordance with Consensus Standard ANSI/ITSDF Safety Standards for Low and High Reach Lift Trucks B56.1-2009, Section 5.3.19: which states An active operator protection device or system, when provided, shall be used.
