Plastic Systems, LLC located at 13950 US Route 68S Kenton, OH; On or about September 5, 2012,	 the employer did not assure that forklift refresher training was provided to an operator involved in an accident. On August 23, 2012 the forklift operator struck another employee from behind. Employees were exposed to additional incidents or near misses.

On August 23, 2012, an employee was struck in the leg from behind while the forklift operator was dropping off a skid	.

Employees were exposed to struck by hazards while operating forklifts with inoperable brakes and master cylinders.

29 CFR 1910.178(p)(1):     Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    a.) Plastic Systems, LLC located at 13950 US Route 68S Kenton, OH; On or about August 23, 2012, the CAT LP Forklift, Model GC15K, Serial #AT81C-00606, was in use during a time in which the brakes were not operational. Employees use forklifts inside the facility to move parts to different locations, including to and from storage locations. An employee was struck from behind in the leg with the forklift while trimming parts causing him to fall and strike his head on the corner of the metal piece that holds the forks. The accident resulted in a contusion to the head and a fractured arm.  b.) Plastic Systems, LLC located at 13950 US Route 68S Kenton, OH; On or about September 5, 2012, the larger CAT LP Forklift #A428191, was in use during a time in which the master cylinder was not operational. Employees use forklifts inside the facility to move parts to different locations, including to and from storage locations and were exposed to struck by and crush hazards while operating and working around the lift.

Employees use forklifts inside the facility to move parts to different locations, including to and from storage locations and were exposed to hazards associated with overloading the lift.    b.) Plastic Systems, LLC located at 13950 US Route 68S Kenton, OH; On or about September 5, 2012, the CAT LP Forklift 1C120-863, was in use during a time in which the nameplate was not affixed to the lift. Employees use forklifts inside the facility to move parts to different locations, including to and from storage locations and were exposed to hazards associated with overloading the lift.
