29 CFR 1910.106(e)(9)(i): Maintenance and operating practices did not control leakage and prevent accidental escape of flammable liquids: On or about February 12, 2014, the storage cabinet in the Tool Room was not maintained in a clean and orderly and in a sanitary condition. Items, such as but not limited to, propane bottles, paints, cleaners, degreasers and other unknown substances were stored haphazardly. One unlabeled container of an unknown substance lay open without cap. Employees were exposed to chemical, burn and health hazards.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include an explanation of the labels received on shipped containers and the workplace labeling system used by the employer; and the safety information found in the safety data sheet, including the order of information, and how employees could obtain and use the hazard information: On February 12, 2014 and times prior to, employees were not trained properly in the new label elements found in the safety data sheets. This condition exposed employee to potential health hazards associates with chemicals.
