29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) On or about March 4, 2013, the employer failed to record a work related injury that occured on January 10, 2013 in their OSHA logs for 2013. The employee was taken off of normal job duties and placed on rectricted activity for approximately 10 weeks.
