29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a)  On or about August 16, 2012, and at times prior, employees were exposed to head injuries as they worked without head protection in areas where shingles and other roofing waste were being thrown from upper levels. 29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    a)  On or about August 16, 2012, employees were exposed to falls of greater than 6 feet while they conducted re-roofing activities without fall protection measures in place.      Association Construction Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 313716268, Citation 1, Item 1, issued January 20, 2010, with respect to a workplace located at 467 Kipling St., Lakewood, CO 80215.  This citation became final on February 1, 2010.    Association Construction Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312142748, Citation 2, Item 1, issued August 4, 2009, with respect to a workplace located at 5694 Quemoy Ct., Aurora, CO 80016.  This citation became final on August 19, 2009.    Association Construction Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 311907620, Citation 1, Item 3, issued September 12, 2008, with respect to a workplace located at 2428 W. 82nd Place, Westminster, CO 80031.
