29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  a) Residential site: On or about June 24, 2016, and at times prior, workers were exposed to falling 14 feet to ground level where the ladder extended less than 18 inches above the landing surface.   Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Residential site: On or about June 24, 2016, and at times prior, workers were exposed to falling hazards while performing roofing work without fall protection at elevations up to 20 feet above ground level.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).  Association Construction Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 894543, citation number 02, item number 002, and was affirmed as a final order on April 17, 2013, with respect to a workplace located at 5941 S Eudora Way, Centennial, CO 80121.  Association Construction Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 615919, citation number 01, item number 003, and was affirmed as a final order on November 12, 2012, with respect to a workplace located at 10660 Stronington St., Littleton, CO 80126.
