29 CFR 1926.451(e)(1): Scaffold platforms more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. (a) Reliant Masonry on or about September 07, 2012, at the address of 2227 Tiffin Ave. Findlay, OH: The employer failed to ensure a ladder or similar means of access was provided to employees using a welded frame scaffold. The employees were exposed to a fall to the ground below when utilizing the frame of a masonry walkthrough scaffold to climb to the platform(s) above.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. a) Reliant Masonry on or about September 07, 2012, at the address of 2227 Tiffin Ave. Findlay, OH: The employer did not ensure employees were protected from a fall while working from a supported scaffold at the east side of the commercial structure. Employees installing brick veneer from a scaffolding platform without standard railing were exposed to a 13.5 fall to the ground below. Reliant Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 314585506, citation number 1, item number 2 and was affirmed as a final order on June 22, 2010, with respect to a workplace located at 8338 Sancus St. Columbus, OH.
