29 CFR 1926.451(e)(6)(i): Integral prefabricated scaffold access frames which were used as scaffold access were not specifically designed and constructed for use as ladder rungs: a) Located along various interior areas of the structure employees were observed gaining access to the scaffold work platforms to perform masonry related work activities by climbing the walk0through scaffold end frames which were not specifically designed and constructed for such use in that rung lengths were 5 inches, rungs were not uniformly spaced and rung spacing exceeded 17 inches, exposing employees climbing end frame legs to fall potentials in excess of 13 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. a) Located along various interior areas of the commercial structure employees were observed working from a self-supported tubular welded frame scaffolding system that was not protected by standard guardrails or personal fall arrest systems exposing employees to fall potentials in excess of 13 feet.
