29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform). a. On or before March 17, 2017, where employees were working from a scaffold, the working level of the scaffold was not fully planked, thereby exposing the employees to a 15 foot fall hazard.

29 CFR 1926.452(c)(5): Brackets used to support cantilevered loads shall: 29 CFR 1926.452(c)(5)(iii): Be used only to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold. a. On or before March 17, 2017, where employees were working from a scaffold, cantilevered side brackets were installed on the outside of the scaffold and used to support a mud trough, exposing employees to fall and crushed-by hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. a. On or before March 17, 2017, where employees were working from a scaffold, the employees were not protected from falling to lower levels, thereby being exposed to a 15 foot fall hazard. Reliant Masonry was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1181807, citation number 2, item number 1 and was affirmed as a final order on December 16, 2016 with respect to a workplace located at 1180 S. Main Street, Englewood, OH 45322 .
