29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)  On or about August 17, 2012, and at times prior, employees were not protected from eye hazards, in that employees were using pneumatic nail guns without eye protection while installing shingles on a roof.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     (a)  On or about August 17, 2012, and at times prior, employees were not protected from fall hazards greater than 6 feet above the ground, in that the employees were roofing a three story apartment building without any using fall protection systems.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
