29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    On or about September 10, 2012, at Highway 211 & Highway 124:    Employees engaged in work activities related to the installation of a storm drain were exposed to a cave-in hazard when they entered an unprotected trench that ranged approximately 7-9-feet wide, approximately 27-feet long, and 5.4-feet deep.
