29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) On or about September 11, 2012 and at times prior Jasper Contractors Inc. as the controlling contractor did not ensure that workers were protected from eye hazards. The roofing crew was using pneumatic nailers to install shingles. They were not using any type of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about September 11, 2012 and at times prior Jasper Contractors Inc. as the controlling contractor did not ensure that workers were protected from fall hazards. The crew was installing shingles on a two story house with eaves over 25 feet high. They were not using fall protection of any kind. Jasper Contractors Inc. was previously cited for a violation of this Occupational Safety and Healtlh standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection 313722787 citation number 1 item 1 issued on January 31, 2011, with respect to a workplace located at 713 Valley View Rd, Woodland Park, CO 80863
