29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 9/19/12, employees were exposed to a fall from height of approximately 12 ft on a steep slope roof while working at the edge of the multi-residential building at the Princeton Place Assisted Living Center while repairing the roof.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): On or about 9/19/12, employees were not trained of the dangers from working at heights from a competent person while working on the roof of Princeton Place Assisted Living Center. Employees were wearing PFAS, however they were not attached to lanyards/ anchor points.
