29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 10, 2021, an employee was exposed to eye hazards when not wearing eye protection while assisting another employee using a nail gun to secure fascia of an apartment building under construction.

29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about June 10, 2021, at an apartment building under construction, employees were exposed to a fall hazard while installing fascia from an aerial lift without wearing a harness with a lanyard attached to the basket.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On�or about�June 10,�2021, at�an apartment building under construction, an employee was exposed to a fall hazard exceeding six feet when working on a third floor balcony and handing materials to employees in an aerial lift without a fall protection system in place.
