29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On or about September 20, 2012, during steel erection activities including but not limited to installing 4 X 4 bracing on a parapet wall on a flat roof approximately 17 feet high, the employer did not ensure all employees on the roof were using a fall protection system/device. LAKE BUILDING PRODUCTS INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.760 (a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311515100, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 9. 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 3220 CENTER RIDGE ROAD, FAIRVIEW PARK, OH 44126.
