Lake Building Products , Inc. worksite located at 36002 Main St., Avon, Ohio: On or about June 20, 2014, the employer did not ensure an employee was protected from fall hazards while working on the top of the 8 inch masonry wall. The employee was assisting in connecting bar joist and did not have the ability to tie off exposing the employee to a fall distance of approximately 22.00 feet on the south side of the commercial structure.     Lake Building Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.760 which was contained in OSHA inspection number 313592941, citation number 1, item number 1 and was affirmed as a final order on February 9, 2010 with respect to a workplace located at Berea, Ohio.
