29 CFR 1926.1424(a)(2)(ii): The employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas. A. Lake Building Products, Inc. worksite located at 36002 Main St., Avon, Ohio: On or about June 20, 2014, the employer did not ensure the swing radius of the crane was barricaded. Ground employees were exposed to the rotating superstructure of the Linkbelt HTC 8650 while performing steel erection activities.

29 CFR 1926.760(b)(3): Each employee was not provided, at heights over 15 and up to 30 feet above a lower level, with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or be provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. A. Lake Building Products , Inc. worksite located at 36002 Main St., Avon, Ohio: On or about June 20, 2014, the employer did not ensure an employee was protected from fall hazards while working on the top of the 8 inch masonry wall. The employee was assisting in connecting bar joist and did not have the ability to tie off exposing the employee to a fall distance of approximately 22.00 feet on the south side of the commercial structure. Lake Building Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.760 which was contained in OSHA inspection number 313592941, citation number 1, item number 1 and was affirmed as a final order on February 9, 2010 with respect to a workplace located at Berea, Ohio.
