29 CFR 1926.502(b)(2):   Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high:  (a) Second Floor of Gym Area Renovation - Employees worked on the second floor, staging equipment in preparation for cutting holes in some concrete block walls to make door way openings.  A section of the guardrail system, 17 feet long, along the eastern edge of the second floor had a top rail but no mid rail, exposing employees to a fall hazard, on or about September 21, 2012.

29 CFR 1926.502(i)(2):  All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time:  (a) First Floor of Gym Area Renovation, Bottom of New South Stairway - The employer made a hole in the floor for work to occur on underground electrical.  The hole had an opening size of 6 feet 6 inches by 5 feet 5 inches and a depth of 48 inches.  The hole had a cover consisting of a single sheet of plywood, partially supported by electrical conduit.  The electrical conduit did not have sufficient capacity to take the weight of a falling employee, and the piece of plywood did not completely cover the hole opening, on or about September 21, 2012.

29 CFR 1926.602(c)(1)(vi):  Industrial trucks in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks; powered industrial trucks with defects or in any way unsafe had not been withdrawn from service until restored to safe operating conditions:  (a) Front of Gym Area Renovation - The outer covering of a hydraulic hose for raising and lowering the boom of the JCB Loadall 508-40/568108 telehandler, had deteriorated in spots, and this powered industrial truck did not get withdrawn from service for repairs, on or about September 21, 2012.
