29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that might expose an employee to injury, the defective or damaged item was not removed from service, and employees were permitted to use it before repairs and tests necessary to render the equipment safe had been made: a) terminal dock, on or about September 25, 2012, the flexible cord of the skilz saw had been cut and the inner wires were exposed. b) west end of terminal dock, on or about September 25, 2012, the receptacle was damaged and was no longer secured to the wall, the conduit had been pulled from the box and the inner wires were exposed.
