Fixtures, lampholders, lamps, rosettes, and receptacles may have no live parts normally exposed to employee contact. Pitt Ohio Express, Inc., 2101 Hardy Parkway, Grove City, Ohio workplace; as of and prior to January 31, 2013, electrical power supply cords for dock lights had damaged insulation exposing employees to contacgt with live parts: a. at the 9 door location, b. between the 5 door and 4 door locations, c. at the 18 door location, d. at the 61 door location, e. between the 62 door and 64 door locations, f. between the 68 door and 70 door locations, g. between the 72 door and the 74 door locations,

29 CFR 1910.305(b)(2)(i): Covers and canopies - Cabinets, boxes, and fittings -- All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Near the old dock tower storage area, there was a receptacle outlet box cover that was bent which did not completely enclose the live parts for the receptacle box.
